Scheduling
To help speed the scheduling process, we ask that you find out if your policy or plan requires pre-authorization or referral. If you need assistance, our experienced staff is available to help. Please call 503-982-4862.
Please remember to bring your insurance card along with a picture ID to your appointment.
Registration
To help speed the registration process when you arrive for your appointment, you may download the registration forms and complete them in advance. If you have been a patient of Silverton Hospital or one of its clinics in the past, please sign the Patient Authorization Form. If you are a new patient, please complete and sign the Patient Information Form (Datos del Paciente).
Results
The results of your procedure may be obtained through your doctor, who will receive a complete report from TDI.

Billing
TDI is an approved provider for most major insurance carriers and health plans. Your claim will be submitted using the complete benefit coverage information you provide, including the insurer's name and address as well as your claim, policy and group numbers. We will also need the name of the policy holder or subscriber.
Workers compensation, automobile accident, or personal injury cases may also require the name and address of your attorney and/or claims adjustor along with your health insurance information.
You will receive two separate bills for your imaging procedure; one for the exam and one for the radiologist's professional services.
Ultimately, you are responsible for the payment of your account. You will receive a monthly statement until the balance is paid. Please contact your insurer if no payment has been made within the first 30 days.
