Our New Legacy

Silverton Health has joined with Legacy Health. Silverton Health facilities now carry the Legacy name. For the Legacy Health patient portal, visit the Legacy Health "MyHealth" page.


Silverton Health's patient portals are secure, web-based, systems for patients to access information about their personal health record or in-patient hospital visits.

After signing up, patients have 24-hour access to their health information and will be able to send their health record to a provider of care.

Our portals provide you with:

  • Laboratory and microbiology results
  • Medication and allergy lists
  • The ability to view upcoming appointments
  • The ability to change demographic information
  • The ability to view and securely transfer the hospital summary of care to the provider of your choice
  • Discharge summary

Two Patient Portals

Why two portals?

Silverton Health's hospital and clinics are on different Electronic Health Records (EHRs) and will require you to sign up for each one. Please note that username and passwords will be different for each portal as they are separate user accounts.

How do I sign up?

Hospital Patient Portal

You can sign up for the Hospital Patient Portal at the hospital’s main admitting desk.

If you did not get an opportunity to enroll in the portal while you were in the hospital, you can use Hospital Self-Enrollment.

Hospital Self-Enrollment

We must have your email on file for you to be able to Self- Enroll in the portal. When you get to the Enrollment screen fill in the following:

  • Name
  • Date of birth
  • Email address
  • Medical record number

You can find your medical record number on your discharge paperwork.

Clinic Patient Portal

If you are currently a patient with our practice and interested in signing up for these services, please contact your physician's office and request the mySilvertonHealth Clinic Patient Portal letter. Once you receive the letter simply click on the Register link at the top of Clinic Patient Portal home page and follow the instructions to sign-up.

Portal support is available Monday–Friday during normal business hours. We will respond to your email requests within two business days.

Resetting My Password

We all forget our passwords from time to time. We understand.

To reset your password for the Clinic Patient Portal, you will need your User ID or Email address associated with your account.

To reset your password for the Hospital Patient Portal, you will need both your Logon Id and your Email Address.

For additional password assistance, please send your support question to portal-support@silvertonhealth.org.

Unlocking My Account

For security reasons and to protect your Personal Health Information, three unsuccessful login attempts will result in a locked account.

On the Clinic Patient Portal, the account lockout will expire in 30 minutes.

On the Hospital Patient Portal, your locked account will need to be unlocked by our staff.

Logging In

If after attempting to reset your password you continue to have problems logging in to either our Hospital or Clinic Patient Portal, please contact us at portal-support@silvertonhealth.org.
